HMT might be faulty even if you are using official Foresight dots. People talk about LED array which needs a replacement from time to time. But this seems to be very rare

But I think the very next step is to try in a different place/environment/equipment (ive heard clubs or shoes sometimes also have a reflection and gives out false readings) and if it still occurs then might have to send in for a repair...

I hit off the toe a lot and if I am striking the ball on the face where one of the dots is located, you will likely only get one dot mode. Try moving the dots to the very edge of the face.

If the HMT picks up less or more than four dots it will roll back to 1 dot mode. So interference from reflections off hosels or shoes could impact it. I was the one who had the failing LED array and all it would do is 1 dot. Foresight has a utility that will collect and send them the images. You might pick up a set of the super reflective dots that are being sold on here to see if they help.

I have seen this in a couple of units. Usually means the phase array is off or the flash is getting weak.

Looks like that is the case. Did your seller say everything was working? You are looking at a costly repair.
