I finally broke down and took lessons. I was told the plane of my swing is causing a lot of my problems. I've somewhat corrected it in the first lesson and I'm already seeing improvements. Can't wait to keep it going and fixing the things that have been frustrating me!

Kudos on taking the lesson, I'm sure they will get you straightened out.
One thing I did notice from the video.....watch your right knee (leg)....for righties it would be our left knee. Notice how you sway and lift it up on your backswing? You are never going to get the proper turn when your right leg is not stable and properly maintained during the backswing. Watch most of the pros and see how their bottom halve is relatively quiet as they COIL the upper body. Now the really long hitters get away with some lower body turning but only because they are maxed out in coiling. Most amateurs are the opposite, we sway or turn our bottom half because it is easier than coiling the top half and we think we are building torque.....torque comes from the top half coiling against the resistance of the bottom half being held steady.....watch your right knee in the video.
Without a proper coil plus a flat swing and your tendency is going to be to push the ball out to the left.

The problem with your swing is actually quite simple; your right should juts out and over the top too quickly on your downswing. If you had your right shoulder further from the target line at impact than it currently is you'd have more of an inside out path, which is what you want.
